1. How do I know if I’m covered for Fertility Care?

Fertility Care is available to Kaiser Permanente members in Southern California with existing infertility, in-vitro fertilization (IVF), or fertility preservation health coverage. We currently do not offer these services to members without coverage. Even with coverage, there is often a cost-share for services, which will be discussed at your initial consultation. For questions about coverage, please call Member Services at 1-800-464-4000 between 7:30 am and 5:00 pm PST. You can also sign in to your kp.org account and view your coverage under Coverage & Costs.

2. I do not have existing Fertility Care coverage. Can I enroll?

Coverage is determined based on your existing plan. Currently, there is no option to separately purchase coverage for infertility, IVF, or fertility preservation.

3. Are there any out-of-pocket costs for procedures available to members or non-members?

Vasectomy reversal is a non-covered procedure available to members and non-members which requires payment in full for the consult and the surgery. This surgical procedure restores the flow of sperm through the vas deferens allowing you to potentially achieve pregnancy through sexual intercourse, artificial insemination, or in-vitro fertilization.

Vasectomy Reversal Fees (subject to change):

  • Consultation – $300
  • Vasectomy Reversal Surgery – $7,500

4. What scenarios might a patient be covered for Fertility Preservation?

Sperm or egg preservation is covered for some patients (coverage exclusions apply) about to undergo a treatment or procedure that is likely to cause infertility. This may include upcoming pelvic radiation, chemotherapy, surgical removal of testes/ovaries, or gender-affirming hormone treatment. The clinical care team will determine your coverage and if you are a candidate for fertility preservation.

5. How do I find out about my financial responsibility?

Determination of patient cost-share is dependent on your coverage and the treatment you are receiving. The clinical care team will work with you to provide cost-share information.
